Requisition Forms

The Requisition Forms course is designed to teach healthcare providers, office staff, and clinic coordinators how to accurately and efficiently complete test requisition forms for Natera’s suite of genetic testing solutions. This course provides a step-by-step guide to understanding what information is required, why it is necessary, and how it ensures the proper processing of genetic tests in a timely manner.

Learners will first explore the purpose of requisition forms, understanding that these documents serve as the official order for a genetic test and a communication tool between the healthcare provider and the testing laboratory. Accurate completion ensures that patient information, clinical indications, and ordering physician details are captured correctly — minimizing delays, avoiding rejected samples, and ensuring compliance with regulatory requirements.

The course will break down each section of a typical requisition form, including patient demographics (name, date of birth, sex), clinical information (pregnancy details, family history, indications for testing), test selection (Panorama, Horizon, Empower, Anora, Vistara, Spectrum™, or Fetal Focus), and provider information (practice name, ordering physician, contact details). Each section will include guidance on how to avoid common errors, such as incomplete checkboxes, missing insurance information, or incorrect ICD-10 codes.

Interactive examples and simulations will allow learners to practice filling out sample forms, including electronic and paper versions. The course will also cover digital requisition submission via online portals, highlighting the benefits of e-forms, barcode tracking, and automated status updates.

Learners will understand how requisition forms tie into sample collection and logistics, including proper labeling of tubes, inclusion of necessary documentation, and shipping requirements to maintain specimen integrity. Additional emphasis will be placed on ensuring that patient consent is documented, particularly for tests that require informed consent, and how to communicate testing plans to patients before submission.

The course also addresses compliance considerations, such as HIPAA privacy requirements, CLIA regulations, and payer requirements for coverage and reimbursement. Participants will gain insight into how properly completed requisitions can speed up insurance verification and improve turnaround times for results.
